IRS Form 433-A: Financial Statement for Individuals

By the Tax Relief Answers editors · Sources checked · How we check

The short answer

Form 433-A, Collection Information Statement for Wage Earners and Self-Employed Individuals, is the detailed financial statement the IRS uses to decide how an individual can pay a tax debt. IRS Publication 1854 describes its purpose that way, and the current revision of the form is June 2026 (checked November 2026 on the irs.gov forms list). It is longer than Form 433-F, and you normally complete it because the IRS or IRS Appeals asked for it. It is a different form from Form 433-A (OIC), which goes with an offer in compromise.

Who files it

Publication 1854 says you may need to complete Form 433-A if you are an individual who owes income tax on Form 1040, who may be a responsible person for a Trust Fund Recovery Penalty, who may be personally responsible for a partnership liability, who is the individual owner of an LLC that is a disregarded entity, or who is self-employed. The form itself tells wage earners to complete Sections 1 to 5 and self-employed individuals to complete Sections 1 and 3 to 7, plus the signature line.

Publication 594 says the IRS may ask for Form 433-F, 433-A or 433-B before approving an installment agreement. The Form 12153 hearing request suggests including Form 433-A if you want Appeals to consider a payment plan, an offer or a hardship status.

Where and how it is submitted

There is no general mailing address. You give the completed form to the IRS employee or office that asked for it, or attach it to your Form 12153 hearing request when you propose a collection alternative there. The form asks you to answer every question or write N/A, and to attach extra pages if you need space. Publication 1854 says the IRS may ask for more proof after it reviews the form.

Steps

  1. Confirm which form the IRS asked for: 433-A, 433-F or 433-A (OIC).
  2. Download the current revision from irs.gov; note that the Spanish version is an older revision.
  3. Read Publication 1854 before you start.
  4. Gather statements for every account, asset, debt and income source.
  5. Send it to the office that asked for it, keep a full copy and record the date you sent it.

FAQ

What is Form 433-A used for?

The IRS uses Form 433-A to get current financial information from a wage earner or self-employed individual and decide how that person can pay a tax debt. The current revision is June 2026, checked November 2026.

Is Form 433-A the same as Form 433-A (OIC)?
