IRS Forms for Tax Debt: Which Form Fits Your Situation

By the Tax Relief Answers editors · Sources checked · How we check

The short answer

Most IRS tax debt situations map to one of about a dozen IRS forms: Form 9465 for a payment plan request, Forms 433-F, 433-A and 433-B for financial statements, Form 656 for an offer in compromise, Form 12153 or 9423 for an appeal, and Form 2848 or 8821 to let someone else deal with the IRS for you. The table below matches each situation to its form and to our page about it. Every form is free to download from irs.gov, and the current revision dates come from the IRS forms and publications list, checked November 2026.

Current revisions and Spanish versions

The IRS updates forms on its own schedule, so an old copy can be out of date. These are the revisions shown in the irs.gov forms list, checked November 2026.

How to use these pages

  1. Find your situation in the first table and open our page for that form.
  2. Read what the form is for and who files it, then open the IRS page or PDF we link. We never host a copy of an IRS form.
  3. Check whether the IRS offers an online option first. For a payment plan or an offer in compromise, it often does.
  4. Download the form from irs.gov on the day you use it, so you have the current revision.

What these pages do not do

They explain what each form is for, who files it and how it reaches the IRS. They do not tell you what to write on any line, because that depends on your facts. For that, the IRS instructions are the source, and a licensed professional (enrolled agent, CPA or attorney) or a Low Income Taxpayer Clinic can look at your case.

FAQ

Do IRS forms cost anything?

No. Every form on this page can be downloaded from irs.gov at no cost. Some programs charge a fee when you apply, such as the $205 offer in compromise application fee listed in the Form 656-B booklet, but the form itself is free.

Where do I get the current version of an IRS form?

From irs.gov. The IRS forms list shows the current revision date for each form; the dates in our table were checked November 2026.

Does the IRS publish these forms in Spanish?

For many of them, yes. The irs.gov forms list includes Spanish versions of Forms 9465, 433-F, 433-A, 433-B, 656-B, 656-L, 12153, 2848, 8821, 8857 and 911, as of November 2026.

Which form do I use to ask for a payment plan?

Individuals who owe $50,000 or less can apply online instead of filing a form, according to the Form 9465 instructions. Form 9465 is the paper request for those who cannot or do not want to use the online tool.
